Abstract Funding Acknowledgements Type of funding sources: None. Background In patients who are refractory to box isolation (BoxI) and have atrial fibrillation (AF) substrate in left inferior wall (LAIW), LAIW is effective, but the procedure highly difficult because connection between coronary sinus (CS) dense. this study, we report four cases persistent AF which simultaneous CS was so effective. Methods Results Case 1 a 76-year-old female with had undergone BoxI failed mitral isthmus block (MIB) creation. However, tachycardia (AT) persisted. 4th session, chemical ablation Marshall vein performed, AF/AT terminated, MIB created. Thereafter, multiple ATs were still inducible by rapid pacing. Because AT seemed be confined LAIW, attempted isolate performing linear right pulmonary (RIPV) posteroseptal annulus (MA) near ostium, parallel along posterior MA. could not achieved. Finally, ostial from atrium (RA) succeeded CS. burst pacing induce although RA maintained rhythm (SR). Rapid AT/AF. 2 78-year-old completed Raid induced AF. cycle length apparently shorter than RA, LAIW. Firstly, performed RIPV ostium During ablation, converted AT, finally returned SR while sustained within This reentrant circling After isolated during SR. isolation. 3 74-year-old BoxI. Complete Isolation of the Left Atrial Posterior Wall (Box Lesion) to Treat Longstanding Persistent Atrial Fibrillation.

INTRODUCTION The left atrial posterior wall has been shown to play na important role in AF pathophysiology. OBJECTIVE Evaluate the efficacy of an ablation strategy designed to completely isolate the LA posterior wall, on top of PV isolation. Methods and Results: 25 pts (72% male age 65 ± 12 years) undergoing AF ablation for persistent or long term persistent AF. Mean AF duration was 11 ± 3 mo...

Catheter ablation for the treatment of persistent atrial fibrillation: Maintenance of sinus rhythm with left atrial appendage and coronary sinus isolation after multiple ablation procedures

Introduction After seminal research demonstrated the pulmonary veins (PVs) and PV antrum contained substrates that can initiate and perpetuate paroxysmal atrial fibrillation (AF) in the majority of patients, catheter ablation with circumferential pulmonary vein isolation (PVI) has become the cornerstone treatment for the management of paroxysmal AF. However, in patients with persistent and long...

Left Atrial Appendage Electrical Isolation and Concomitant Device Occlusion to Treat Persistent Atrial Fibrillation

Catheter ablation has transformed the treatment of atrial fibrillation (AF); however, the rhythm outcomes in treating persistent AF are variable, often requiring multiple procedures to maintain long-term freedom from atrial arrhythmias. Electric isolation of the pulmonary veins (PVs) is central to catheter ablation strategies; however, focal sources and other arrhythmic substrates outside the P...

Outcome of stand-alone thoracoscopic epicardial left atrial posterior box isolation with bipolar radiofrequency energy for longstanding persistent atrial fibrillation

INTRODUCTION Catheter ablation of longstanding (> 1 year) persistent atrial fibrillation (AF) is associated with poor outcome. This might be due to remodelling and fibrosis formation, mainly located in the posterior left atrial (LA) wall. Therefore, we adopted a thoracoscopic epicardial box isolation of the posterior left atrium using bipolar RF energy with intraoperative testing of conduction ...

Pulmonary vein isolation for paroxysmal and persistent atrial fibrillation.

BACKGROUND The pulmonary veins (PVs) have been demonstrated to often play an important role in generating atrial fibrillation (AF). The purpose of this study was to determine the safety and efficacy of segmental PV isolation in patients with paroxysmal or persistent AF. METHODS AND RESULTS In 70 consecutive patients (mean age, 53 +/- 11 years) with paroxysmal (58) or persistent (12) AF, segme...
